HOW TO APPLY FOR LATE REGISTRATION of BIRTH
This is the case of my mother late registration of her BIRTH CERTIFICATE.
War came suddenly to the Philippines on Dec. 8, 1941(Dec. 7 US TIME). My mother was born November 9, 1941. So by that time my Grandparents did not think of registering my mother because of the war going on. But my mom was baptized at the San Pedro Cathedral, that is her only proof of her extension.
I went to Sangguniang Panglungsod to get information how to late registered my mother's birth. They give me a small paper stating all the requirements:
I accompanied my mother to get all her requirements since my mom is already 71 years old.
First we get this:
We have problems with her name, her baptismal only have LORETO GONZALES. Since she is using two names on her other certificates, Ma. Loreto and Maria Loreto, she is asked to use only one to reflect to her new baptismal certificate according to church in-charge.
So she chooses MARIA LORETO GONZALES indicated on her new baptismal certificate. We pay PHp150 and waited for 1 month processing.
I get a copy of the AFFIDAVIT OF TWO DISINTERESTED PERSONS from DOCSTOC. I just change some of its statement applying for my mom case. Have it signed by our two neighbors who knows my mom. And we went to Sangguniang Panglungsod to have it notarized.
The next day, we went to NATIONAL STATISTICS to get a copy of my mom NEGATIVE CERTIFICATION of her registration. We pay PHp140.
After 1 month we get my mom Baptismal Certificate. All documents ready for checking and passed it to the LATE REGISTRATION WINDOW at Sanggunian Panglungsod.
Then they give us a CLAIM STUB.

***UPDATE for my Mother's late registration, it take us years to process it because there is a conflict between the year of her birth date and her sisters birth date. Supposedly, my aunt birthday is May of 1940 and my mother is 1941. But as we check their family registration, my aunt use 1941 instead of 1940. So, we have to ask our family attorney to check and what will be our next move in order to arrange properly all both their details. Up to now, honestly, we cannot already change the year my aunt is using because all of her documents (GSIS, SSS, PHILHEALTH, etc) she used the 1941 year instead of 1940.
Our attorney suggest, is to make another late registration and use the year 1942, but the other concern is my mother BAPTISMAL CERTIFICATE (the only living proof that she's born in year 1941).

- CEDULA or Community Tax Certificate
- Certificate of Late Registration from your Barangay
- Barangay Clearance
- Marriage Certificate
- Baptismal Certificate
- Negative Certification from NSO
